Applying for a U.S. Passport

First-Time Passport Applications :
** The regular processing time is about 3 to 4 weeks.
** If your ID does not match your name that is on your proof of citizenship document, please bring proof of your name change. Documents must be original. (ex. Marriage Certificate, Court Order)
** Original documents will be returned to you after review of your application.

Adults : (age 16 and older)
** All applicants must apply in person.
** Proof of U.S. citizenship (original + 1 photocopy)
** Valid ID (original + 1 photocopy)
** One passport photo

** White background /2?×2? in size / no eye glasses / no uniforms
** Form DS-11
** Complete online and print out single-sided.
** You must provide a Social Security number, if you have been issued one.
** One “Japan Post Letter Pack envelope” (self-addressed)

** Letter Pack LIGHT – 360 yen, traceable, signature not required
** Letter Pack PLUS – 510 yen, traceable, signature required
** For persons using an APO/FPO address, bring one self-addressed envelope
** Passport Fee

Proof of Citizenship

Proof of U.S. Citizenship can be demonstrated by the following. Military ID cards and social security numbers are NOT proof of citizenship.

** A full validity U.S. passport (currently valid or expired)
** Certified U.S. birth certificate (should have parents’ names, a seal, registrar’s signature, and the date the certificate was filed with the registrar’s office – hospital birth certificates are not acceptable)

** *Birth Abstracts may not be acceptable.
** Consular report of Birth Abroad (Form FS-240/FS-1350)
** Certificate of Naturalization
** Certificate of Citizenship

Passport Renewals

** Processing time is about 3 to 4 weeks.
** Original documents will be returned to you after review of your application.

Adults (age 16 and older) application in person :
** Current or expired passport
** If the most recent passport was issued within the last 15 years and when you were age 16 or over, complete form DS-82 online and print out single-sided.

** One valid proof of ID
** One passport photo
** White background / 2×2 in size / no eye glasses / no uniforms
** One “Japan Post Letter Pack envelope” (self-addressed)

** Letter Pack LIGHT: 360 yen, traceable, signature not required
** Letter Pack PLUS : 510 yen, traceable, signature required
